The registered nurse coordinates the plan of care with the physician and hospice medical director through initial and ongoing nursing assessment.
As a member of the hospice care team, the social worker provides initial and on going psychosocial assessments.
The physicia is responsible for identifying the patient's need for end-of-life care, and making a referral.
The home health aide assists the patient and family with personal care needs and light housekeeping.
The hospice chaplain provides spitirual support to the patient and family as needed. The focus of counseling is more spiritual then religious.
The hospice volunteer, who are required to attend volunteer training, provide comanionship and support to the patient and family.
